OCR description
The covered entity (CE), Stanley Street Treatment and Resources, reported that it experienced a hacking attack that affected the protected health information (PHI) of 45,785 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, financial information, diagnoses/conditions, medications, and other treatment information. The CE notified HHS, affected individuals and the media. In response to the breach the CE implemented additional technical safeguards.
