OCR description
The covered entity (CE), NewYork-Presbyterian, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 12,000 individuals. The PHI involved included names, addresses, health insurance and financial information, passport information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE retrained staff in its requirements to protect and secure PHI and implemented additional administrative safeguards.
