OCR description
The covered entity (CE), Epic Management reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 10,050 individuals. The CE notified HHS, affected individuals, and the media. After filing the breach report, the CE ceased all operations and the investigation was therefore closed.
