OCR description
The covered entity (CE), Yakima Neighborhood Health Services, reported that a workforce member inadvertently sent the protected health information (PHI) of 2,689 individuals to the wrong recipient. The PHI involved included names, dates of birth, other identifiers, and treatment related information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided additional privacy and security training to its workforce members, implemented additional technical safeguards, and revised its policies and procedures.
