CCA Health Plans of California, Inc. f/k/a Vitality Health Plan of California, Inc.

The covered entity (CE), CCA Health Plans of California, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 14,631 individuals. The PHI involved included names, dates of birth, Social Security numbers, passport information, diagnoses, medications, lab results, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit protection services and implemented additional administrative, technical, and security safeguards to prevent similar incidents.
