OCR description
The covered entity, Innovative Service Technology Management Services, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 2,654 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license and Social Security numbers, and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE strengthened its administrative, technical, and security safeguards.
