OCR description
The covered entity (CE), Consumer Directed Services in Texas, reported that its business associate (BA) experienced a ransomware attack that compromised the protected health information (PHI) of 56,728 individuals. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE terminated its business relationship with the BA.
