OCR description
The covered entity (CE), NYC Health + Hospitals, reported the loss of a hard drive for a visual field-testing device which contained the protected health information (PHI) of 2,174 individuals. The PHI involved included names, dates of birth, and treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards and retrained workforce members on methods to better protect PHI.
