Hanger Prosthetics and Orthotics

The covered entity (CE), Hanger Prosthetics & Orthotics dba Hanger Clinic, reported that an employee of its business associate (BA) was the subject of an email phishing attack that compromised the protected health information (PHI) of 1,354 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, claims information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented new technical safeguards and retrained its workforce on email security.
