Robert S. Miller, LICSW, ACSW, PLLC, the covered entity (CE), reported that he experienced a cybersecurity incident that compromised the protected health information (PHI) of 902 individuals. The PHI involved included names, dates of birth, mailing addresses, email addresses, phone numbers, Social Security numbers, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. Staff were retrained.
