Three Rivers Provider Network

The business associate (BA), Three Rivers Provider Network, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 4,725 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, health insurance information, diagnoses, medications, and other treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative and technical safeguards and retrained its staff.
