OCR description
The covered entity (CE), Prairie Lakes Healthcare System, reported that its business associate (BA) experienced a ransomware incident that affected the protected health information (PHI) of 1,164 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, and financial and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA revised its policies and procedures and implamented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Breach Notification Rule.
