OCR description
The business associate (BA), Verisma Systems, reported that the protected health information (PHI) of 1,586 individuals was viewable on its website. The PHI involved included names and dates of birth. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services and strengthened its administrative and technical safeguards. Workforce members were retrained.
