OCR description
MJ Care, the covered entity and business associate (CE/BA), reported that an employee was subjected to an email phishing scheme that compromised the protected health information (PHI) of 1,832 individuals. The PHI involved included names, Social Security numbers, dates of birth, financial information, diagnoses, medications, and other treatment information. MJ Care notified HHS, affected individuals, and the media. In response to the breach, MJ Care implemented additional administrative and technical safeguards.
