OCR description
The covered entity (CE), L. Knife & Son Employee Benefits Plan reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 4,082 individuals. The type of PHI affected included names, addresses, birthdates, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative, technical, and security safeguards.
