The Kelberman Center

The Kelberman Center, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 3,501 individuals. The PHI involved included names, addresses, birthdates, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, law enforcement officials, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. The CE also applied sanctions and retrained staff.
