OCR description
The covered entity (CE), NR Florida Associates reported that it experienced a ransomware attack that affected the protected health information (PHI) of 6,250 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. During OCR’s investigation, the CE ceased operations and therefore our investigation was closed.
