OCR description
Live Oak Surgery Center, the covered entity (CE), reported that several employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 5,264 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, financial information diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional safeguards and retrained its staff on email security.
