OCR description
The covered entity (CE), Lantern Hill Retirement Community, reported that a former employee impermissibly accessed a company application that contained the protected health information (PHI) of 528 individuals. The PHI involved included names and addresses. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
