OCR description
The covered entity (CE), BayCare Clinic, reported that its business associate (BA) used digital tracking technology that may have compromised the protected health information (PHI) of 134,000 individuals. The PHI involved included names and treatment information. The CE notified HHS and affected individuals; the BA notified the media and posted substitute notice on its website.
