OCR description
1st Franklin Financial Corporation Master Welfare Benefit Plan, the covered entity (CE), reported that employees experienced an email phishing incident that resulted in the deployment of ransomware that compromised the protected health information (PHI) of 3,039 individuals. The PHI involved included names, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE strengthened its technical safeguards and retrained its workforce on identifying potential phishing schemes.
