OCR description
The covered entity (CE), Chico Immediate Care Medical Center, reported that a workforce member impermissibly accessed the protected health information (PHI) of 3,780 individuals. The PHI involved included names, addresses, and phone numbers. The CE ceased operations and is no longer in business; the investigation was therefore closed.
