OCR description
The covered entity (CE), City of Cleveland Health Plan, inadvertently posted the protected health information (PHI) of 15,206 individuals to a public website. The PHI involved included names, Social Security numbers, medication information, and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards.
