Valley Care Home Health Services, Inc.

The covered entity (CE), Valley Care Home Health Services, reported that an intruder broke into its office and took a safe which stored encrypted back up hard drives containing the protected health information (PHI) of 2,674 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, local law enforcement, and provided substitute notice. In its mitigation efforts, the CE revised its policies and procedures and implemented additional physical safeguards. Staff were retrained to better protect PHI.
