Cedar Oaks Surgery Center

The covered entity (CE), Cedar Oaks Surgery Center, reported that an employee was subjected to an email phishing scheme that compromised the protected health information (PHI) of 794 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards. Staff were retrained on email security.
