OCR description
The covered entity (CE), Institute for Hormonal Balance, reported that a document that contained the protected health information (PHI) of 597 individuals was stolen from an employee’s vehicle. The PHI involved included names and medical procedures performed. The CE notified HHS and affected individuals. In its mitigation efforts, the CE moved all patient data to secure online storage, updated training materials, and retrained employees. OCR provided technical assistance regarding the HIPAA Rules.
