OCR description
The covered entity (CE), Hospice of the Valley, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 3,840 individuals. The PHI involved included names, addresses, health insurance information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and provided complimentary credit monitoring services to affected individuals.
