OCR description
Frank R. Laurri, M.D. and Associates, (the covered entity (CE) reported that it experienced a cyber-attack the compromised the protected health information (PHI) of 2,673 individuals. The PHI involved included names, addresses, phone numbers, dates of birth, health insurance information, and other identifying and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the implemented additional administrative and technical safeguards to mitigate risk.
