OCR description
The covered entity, NR Pennsylvania Associates dba Retreat Behavioral Health, reported that experienced a ransomware attack that compromised the protected health information (PHI) of 14,335 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth and medical and treatment information. The CE ceased operations and therefore, OCR closed its investigation.
