OCR description
The covered entity (CE), Nelson Pharmacy Consulting Services, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 13,752 individuals. The PHI involved included names, addresses, dates of birth, and diagnoses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards.
