Highmark, the covered entity (CE), reported that an employee was subject to an email phishing scheme that effected the protected health information (PHI) of 239,039 individuals. The PHI involved included names, dates of birth, driver’s license numbers, addresses, Social Security numbers, claims and financial information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. All staff were provided additional cybersecurity training including instruction on proper identification of fraudulent email communications.
