OCR description
Integrative Dentistry, the covered entity (CE), reported that an employee impermissibly accessed the protected health information (PHI) of 2,454 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, phone numbers, email addresses, financial and health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE imposed sanctions on the workforce member and implemented additional administrative and technical safeguards to protect PHI.
