OCR description
The covered entity (CE), Troy Roeder DDS, reported that employees impermissibly shared their passwords which compromised the protected health information (PHI) of 1,127 individuals. The PHI involved included names, addresses, and treatment information. In response to OCR’s investigation, the CE implemented additional technical and physical safeguards, revised its policies and procedures, and retrained its employees on the HIPAA Privacy Rule. OCR provided technical assistance to the CE regarding its relevant Security Rule obligations.
