OCR description
The covered entity (CE), Henrico Doctors Hospital, reported that a workforce member failed to use the blind copy function and subsequently sent the protected health information (PHI) of 990 individuals to unauthorized individuals. The PHI involved included names, email addresses, and treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE retrained the employee responsible for this breach. OCR provided technical assistance to the CE during this investigation.
