OCR description
Harrisburg Medical Center, the covered entity (CE), reported that it experienced a hacking incident that compromised the protected health information (PHI) of 147,826 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, financial information, diagnoses, medical conditions, lab results, medications, and digital/electronic signatures. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical and security safeguards.
