OCR description
The business associate (BA), eMDs, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 625 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, medications, and other treatment information. The BA notified HHS and the affected entities. In response to the breach, the BA implemented additional system monitoring and technical safeguards.
