OCR description
The covered entity (CE), Child Clarity Guidance Center, reported that it erroneously emailed an electronic file containing the protected health information (PHI) of 531 individuals to another CE. The PHI involved included names, dates of birth, and other identifiers. The CE notified HHS and the affected individuals of the breach. In response to the breach, the CE implemented additional administrative and technical safeguards. Staff were retrained on email security precautions.
