OCR description
The covered entity (CE), Denver Public Schools Medical Plans, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 35,068 individuals. The PHI involved included names and Social Security numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. To mitigate harm, the CE offered complementary credit monitoring services and implemented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Rules.
