OCR description
TThe Chautauqua Center, the covered entity (CE), reported that it’s business associate (BA) inadvertently made the protected health information (PHI) of 745 individuals viewable over the Internet. The PHI involved included names, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts the CE switched to another electronic medical record service provider.
