OCR description
The covered entity (CE), Community Health Centers of Greater Dayton, reported that a workforce member failed to encrypt an email that contained the protected health information (PHI) of 516 individuals. The PHI involved included names, dates of birth, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
