OCR description
The business associate (BA), EPIC Management, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 1,190 individuals. The PHI involved included names, member identification numbers, and health insurance and financial information. The BA notified HHS, affected individuals, and the media. In response to the breach the BA implemented additional technical safeguards and retrained its workforce members to better protect its PHI. OCR provided technical assistance to the BA regarding the HIPAA Rules.
