OCR description
The covered entity (CE), Colquitt Complete Care, reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 78,759 individuals. The PHI involved included names, dates of birth, phone numbers, addresses, diagnoses, Social Security and drivers’ license numbers and health insurance and other treatment information. Colquitt notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative, technical, and security safeguards.
