OCR description
The covered entity (CE), Bone & Joint, S.C., reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 105,094 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring services and implemented additional administrative and technical safeguards.
