OCR description
The California Department of Health Care Services, the covered entity (CE), reported that its business associate (BA) mailed the protected health information (PHI) of 6,460 individuals to the wrong recipients. The PHI involved included names, birthdates, Social Security numbers, addresses, and clinical information. The CE notified HHS, affected individuals, and the media. In response to this incident, the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
