OCR description
The covered entity (CE), Healthy Options dba Postal Prescription Services – Kroger, reported that employees impermissibly shared the protected health information (PHI) of 82,466 individuals with unauthorized recipients. The PHI involved included names and email addresses. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better sensitive data.
