OCR description
The covered entity (CE), AllCare Plus Pharmacy, Inc., reported that it an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 757 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, claims and financial information, diagnoses/conditions and medications. The CE notified HHS and the affected individuals. To mitigate harm to affected individuals, the CE offered complimentary credit monitoring and identity protection services and implemented technical safeguards.
