Atlantic Dialysis Management Services, the business associate (BA) reported that it experienced a cybersecurity incident that compromised the protected health information (PHI) of 745 individuals. The PHI involved included names, dates of birth, Social Security numbers, drivers’ license numbers, financial and claims information, health insurance information, and other treatment information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative, technical, and security safeguards.
