OCR description
The covered entity (CE), WellBe Senior Medical, reported that its business associate (BA) experienced a cybersecurity incident that compromised the protected health information (PHI) of 19,567 individuals. The PHI involved included names, addresses, dates of birth, lab results, medications, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services implemented additional administrative and technical safeguards.
