California Physicians' Services d/b/a Blue Shield of California

The covered entity (CE), California Physicians’ Services dba Blue Shield of California, reported that its business associate’s (BA) subcontractor suffered a hacking attack affecting the protected health information (PHI) of 61,788 individuals. The PHI involved included names, dates of birth, and addresses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE severed its connection with the servers affected by the breach, implemented additional technical safeguards, and provided complimentary credit monitoring services to affected individuals.
