OCR description
The covered entity (CE), Gretna Family Health, reported that it was the victim of a ransomware incident that affected the protected health information (PHI) of 845 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, claims information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative and technical safeguards.
